How the Referral Process Works
Contact Us
Reach out by phone or email to let us know you are interested in services. We will respond promptly to discuss your needs.
Initial Consultation
We will schedule a conversation to learn about the individual's goals, needs, and current supports to determine the best fit.
Service Authorization
We work with the individual's supports coordinator to obtain the necessary ODP service authorization and documentation.
Support Begins
Once authorized, we match the individual with a qualified support staff member and services begin on an agreed schedule.
